Is facial redness causing you to feel blue?

There are several treatments available to combat rosacea, the chronic skin condition which leaves sufferers with redness and flushing on the nose and cheeks.

“Rosacea most often affects middle-age and older adults,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

And although rosacea can develop in people of any skin color, it tends to occur most frequently and is most apparent in people with fair skin.

The Mandelic acid peel, composed of 40 percent mandelic acid, is one treatment offered at Eterna MedSpa that is ideal for those battling intermittent or permanent redness, Malinowski said.

“This is a soft acid to help reduce the appearance of occasional to permanent skin redness and general improvement in skin color and texture and pore size,” she said.

Laser therapy can also be used to help with redness in the cheeks, nose and chin and to remove the visible blood vessels (telangiectasia) that come from rosacea.

Eterna MedSpa also carries Avene’s Antirougeurs.

“The Antirougeurs line is targeted as redness relief,” Malinowski said. “It is clinically proven to reduce the redness associated with rosacea.”

The Antirougeurs Dermo-Cleansing fluid is a gentle, soothing formula which cleanses and comforts the skin while reducing heat sensations. The Antirougeurs Redness Relief soothing cream contains an SPF of 25.

“The cream moisturizes, protects, neutralizes and alleviates existing redness with a broad spectrum UVA/UVB protection,” Malinowski said.

Eterna MedSpa facials can also do wonders to battle facial redness.

“If someone has rosacea they can come in for a mini facial and we can use the Antirougeurs line along with all our other calming products to give the patient a calming and soothing treatment to cut the redness,” Malinowski said.

Lines and wrinkles can be a thing of the past with exfoliation treatments.

“When we were younger our dead skin cells sloughed off left and right on its own daily,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center. “But as we age, the sloughing process slows down.”

To aid in removing those dead skin cells, Eterna offers several products and exfoliation treatments. The latter includes both microdermabrasion and chemical peels.

With microdermabrasion, the exfoliation is done using a powerful electronic vacuum which sprays high-grade micro crystals across the surface of the skin through a pressurized wand in order to slough off the damaged layers. A soothing hydrating mask is applied after to help calm the skin.

As for chemical peels, there are several types available to exfoliate the skin.

“Glycolic and salicylic to name a few,” Malinowski said. “A consultation with your esthetician will determine which peel will be best for you.”

Eterna MedSpa also offers a more invasive form of exfoliation with the Microlaser Peel.

Through the cosmetic treatment, the laser ablades the top layers of the dead skin cells. During the healing process, new cells form, improving the skin’s overall appearance.

How often should you exfoliate?

“It depends on your skin,” Malinowski said. “If you are more sensitive you shouldn’t exfoliate too often. If you have a thick stratum corneum because you have lots of sun damage you can exfoliate more often.”

The solution to younger looking skin can be found in three steps.

The first and probably simplest action you can take?

“Keep your face out of the sun,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

Secondly, make sure to use products that will protect your skin. For instance, when you are out for extended periods of time, make sure to use an SPF of at least 30.

“And reapply, reapply, reapply every hour to hour and a half,” Malinowski said.

The daily application of a Vitamin C serum or products that contain Vitamin C can also help prevent skin aging.

“Vitamin C Tocopherol is used in the morning before you put on your moisturizer and SPF.

Vitamin C protects your skin from free radical breakdown which can cause fine lines and wrinkles,” Malinowski said.

As people age, skin cell turnover rates start slowing down.

“The dead skin cells don't slough off like they did when we were younger,” Malinowski said.

Products containing both glycolic and alpha hydroxy acid can help keep that turnover active in sloughing off the dead skin cells.

“Otherwise the dead skin cells just build up and any product you put on your skin sits on top of those dead skin cells and doesn't penetrate into the skin. Also, the lines on your face look deeper with that build-up,” Malinowski said.

Using a product that has retinaldehyde at night can also help, she added.

The third step to younger looking skin is just a phone call away.

“You should be visiting your esthetician on a regular basis so she can guide you as far as treatments and products,” Malinowski said.

Take time to honor your skin during National Acne Awareness Month

For some, acne is an inevitability.

And while it’s most common in adolescents and young adults, the older crowd isn’t immune.

June has been designated as National Acne Awareness Month in order to raise awareness about the common skin problem and highlight the treatments and products available to combat it.

One effective treatment is a facial. And at Eterna Med Spa, clients will find one that is specifically geared toward breakouts. The pore-cleansing acne facial includes a skin cleansing, exfoliation, steaming, extractions, as well as a mask and moisturizer.

Chemical peels can also help address the concern.

The Obagi Blue radiance peel, a salicylic acid-based peel “works really well for acne,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

But, she noted, as with all peels, in order to see a significant result they must be done in a series of three to five.

“It’s like working out. You can’t work out one time and expect to see toned muscle. You need to stay at it. This also rings true for treating acne,” Malinowski said.

A daily skin care regimen can also help battle acne.

Experts recommend cleansing the skin each morning with a foaming gel cleanser, and then applying a non-comedogenic topical and SPF. A face cleanse before bed is also imperative to wash off the bacteria accumulated during the day.

Makeup should also be non-comedogenic.

Those with severe acne can referred to a dermatologist who may prescribe some form of medication, such as the topical cream Retin-A.

Don't stop your Obagi in the Summer!

Can all patients use the Obagi Nu Derm System (with tretinoin and hydroquinone) during the summer? 

Absolutely!  In fact, it is especially important to use your Nu Derm products during the summer when sun exposure is at its highest and skin’s susceptibility to sun damage is at its peak.  If there was ever a time to correct and protect your skin with Nu Derm, SUMMER is that time!

One of the misconceptions regarding tretinoin is that it thins the skin and makes skin more susceptible to sun burn.  Experts would disagree!  Although this can appear true during your skin’s initial adjustment period (as skin can experience redness, sensitivity and irritation), with continued use of tretinoin, the epidermis is actually thickened and strengthened. While your skin is adjusting to your retinoic acid/tretinoin – as ALWAYS -  it is important to use a SPF like one of your favorites from Obagi’s Sun Shield line that includes Sun Shield MATTE.  With proper protection, you can experience the benefits of great skin care, all year long.

University of Pittsburgh Medical Center hears this question a lot:

“Many patients ask if they should take a break from using their skincare products during the summer months.  This stems from concerns with sun exposure and increased outdoor activities.  However, I have often times found that patients that need the NuDerm product line most are those with pigmentation issues such as melasma or post-inflammatory hyperpigmentation.  These are conditions that typically flare and worsen during hot, sunny weather.  In order for patients to have adequate control over these conditions, I recommend that they continue to use their NuDerm products as instructed.  This goes hand in hand with sun avoidance during the peak hours of 10 am to 2 pm, wearing a hat when outdoors, and reapplying their sunscreen after sweating, swimming, or every 2 hours while outdoors.  Furthermore, while ultraviolet ray B (UVB, the burning ray) increases during the summer, UVA (the aging ray) remains constant all year.  Therefore it is imperative to use a daily, broad-spectrum sunscreen such as Obagi Sun Shield, daily.  I recommend it even while driving or working under fluorescent lights, as these are sources of ultraviolet light exposure that most patients do not realize.”

Doris Day, MD, is also renowned for her expertise and is a leading expert on dermatology and aesthetic dermatology. Dr. Day is affiliated with Lenox Hill Hospital in New York City and is a clinical associate professor of dermatology at the New York University Langone Medical Center where she was presented with the award for Dedication and Excellence in the Teaching of Dermatology. According to Dr. Day with regard to retinoid usage and sensitivity:  "This is one of the biggest retinoid myths," says Doris Day, MD, (a tretinoin user herself). "The ingredient itself is sensitive to sunlight, which is why you should apply it before bed at night. A retinoid shouldn't make your skin any more vulnerable to UV rays than it would be after buffing away dead skin with a face scrub. Summer is actually a good time to start a retinoid: Humidity makes your skin less likely to dry out as it adjusts. Of course, apply sunscreen (SPF 30, at least) as diligently as you always do."

Is there anything extra I can do to protect my skin during the summer months when sun exposure is at its peak?

In addition to using a sunscreen, incorporate a topical antioxidant into your morning skin care routine. Antioxidants help in the fight against free radical and sun damage.  They help stabilize collagen, which helps reduce the aging effects the sun inflicts on our skin. Chemical peels can be a great way to fight the skin’s aging process.

In particular, glycolic facial peels, or alpha-hydroxy acid chemical peels, are ideal for those with visible signs of aging, such as brown spots, also called hyperpigmentation, uneven skin texture, and photaging, which is the damage done to the skin over a person’s lifetime.

“It is an all over skin tone enhancer,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

With the peels, skin is cleansed and a chemical solution is applied to the face which removes the top layer of the skin, revealing a fresh, new layer.

The result? Softer, smoother and more radiant skin.

The alpha-hydroxy acid in the peel is usually derived from natural sources, such as sugar cane, fruit and milk sugars.

Results, which include improved skin texture and tone, are typically best when the glycolic peel is done in a series of 1 to 4 a year.

Chemical peel procedures take less than an hour to perform, with minimal discomfort during the treatment.

Facials, peels can be solution to mild acne

Struggle with acne?

You’re not alone. According to the American Academy of Dermatology, acne is the most common skin condition in the United States, affecting up to 50 million Americans annually.

But there is no one-size-fits-all solution.

“Acne is tough. What works for one person may not work for the next,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

However, mild blemishes and breakouts can be combatted with a facial customized for acne-prone skin.

At Eterna MedSpa, the treatment includes skin cleansing, exfoliation, steaming and pore extractions.

“We then mask with a clay mask and end with a moisturizer with SPF,” Malinowski said.

Clients are also asked about their skin care regimen during the facial.

“We like to see what their daily routines are and hopefully figure out what is causing the acne,” Malinowski said.

Following the facial, a foaming cleanser and either a retinol product, salicylic acid or benzoyl peroxide is suggested to help battle the breakouts.

“We can also do a series of salicylic peels for acne that helps to clear them up,” Malinowski said.

The peel is ideal for clients with oily, blemish-prone skin. It also has an exfoliating effect that can diminish the look of whiteheads, blackheads, papules, and pustules.

Those with severe acne are referred to a dermatologist who may prescribe some form of medication.

Brazilian wax key to long-term smooth skin

Want to get rid of the hair down there?

Ditch the razor and let a Brazilian wax do the trick.

While the popular hair removal method may not be the most pleasant experience, waxing does have longer-lasting effects than shaving.

And the process of going bare in the nether regions gets easier, according to Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

“My clients that do it on a regular basis say the pain lessens each time they wax,” she said.

Clients can prepare for the spa treatment by making sure pubic hair is no longer than a 1/4-inch to 1/2-inch.

“If it is too short the wax will not be able to grip the hair follicle,” Malinowski said.

During the procedure, a zinc oxide wax is used for the outer area, which means less irritation and redness, Malinowski said.

A hard wax is used for lifting the hair in the more sensitive inner region.

While the effects of shaving may only last for a few days, waxing can last about six weeks.

“This is not to say that someone can’t go longer or shorter,” Malinowski said. “It really depends on the person’s hair growth.”

Clients may experience some redness after the procedure. Other than that, they can enjoy their smooth, hair-free skin for weeks at a time.

“In the old days they used to do vein stripping which was a more invasive treatment and the patient was then laid up for several days afterward,” said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

Now, thanks to advancements in technology, there is an alternative to surgical stripping.

Endovenous laser treatment “takes less time and is less invasive for the patient,” Malinowski said.

Unlike vein stripping, EVLA permanently shrinks and collapses the abnormal vein, thereby leaving it in place without surgically removing the vein. Laser energy is delivered through a tiny laser fiber inserted into the targeted varicose vein. The EVLA procedure is performed through a tiny nick in the skin to avoid scarring. No stitches are needed to close the small nick. The laser fiber is positioned using ultrasound guidance.

“We administer local anesthesia to completely numb the vein before treatment so the treatment is virtually painless,” Malinowski said.

The FDA-cleared, minimally-invasive technique treats symptomatic varicose veins with excellent clinical and aesthetic results. Complications are minimal in the hands of an experienced Vein Specialist.

First, she recommends investing in a good skin care product line.

“See an esthetician so she can guide you as to treatments and products,” Malinowski said.

She also recommends a yearly visit to a dermatologist for a mole check.

The third, but most important step, Malinowski said is to stay out of the sun.

“I'm not saying you have to become a hermit and never come out of your house but be sensible,” she said. “Don't bake yourself in the sun with baby oil. If you work in the sun or play sports in the sun make sure you use a good SPF of at least 30.”

And when it comes to the sunscreen, “reapply, reapply, reapply,” Malinowski said.

“SPF is not a one and go. When you are out in the sun you need to reapply every couple of hours,” she said. “And keep in mind when an SPF says it’s waterproof it is not. Nothing is waterproof.”

The Importance of a Daily SPF

If you’re thinking of heading out of the house without wearing sunscreen, think again.

According to Skin Cancer.org, between 40 and 50 percent of people who live to the age of 65 will have either basal cell carcinoma or squamous cell carcinoma at least once.

One way people can protect themselves from harmful ultraviolet rays is by using an SPF sunscreen, said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

And the amount of SPF, or sun protection factor, is important.

“Wear a daily SPF of at least 30,” Malinowski said. “Remember to reapply when you are out in direct sunlight every couple of hours. If swimming, reapply after you dry off. Nothing is waterproof.”

Reapplying is key, Malinowski said.

“It isn't a once and go,” she said. “SPF needs to be reapplied every two hours, especially if you are out in direct sunlight all day. I cannot tell you how many people come in and are complaining about their hyperpigmentation but only put it on in the morning and think it is going to last all day.”

Monthly self-exams of the skin and yearly dermatologist appointments are also recommended.

Want to relax both mind and body? Melt your stress away with an aromatherapy wrap.

Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center, said the essential oils used during the treatment can help induce a feeling of calm.

“Aromatherapy wraps are relaxing, like a massage,” she said. “We ask the client how are they feeling. If they feel they need to relax we use lavender or tranquility, which is a blend of oils to relax the mind. If they feel like they are achy or have a headache we use peppermint oil.”

During the spa treatment, the body is first dry brushed to open up the pores. Next, aromatherapy oils are massaged into the skin. Clients are then wrapped in plastic and a solar blanket which warms up the body and allows the oils to penetrate better, Malinowski said, and treated to a de-stressing neck and scalp massage.

The procedure, which lasts about 45 minutes, results in calming muscle tension and a sense of relaxation.

“When clients are unwrapped we tell them to let the oils continue to work throughout the day, Malinowski said.

How often can the aromatherapy wraps be done?

“Most people come in every couple of months or every month,” Malinowski said.

A non-surgical anti-aging treatment is fast rising in popularity.

Platelet Rich Plasma therapy uses a client’s own blood plasma to rejuvenate the skin of the face and neck, said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

“Improvements have been noted in all of the following: skin tone/tightness, horizontal lines, texture and smoothness, folds, pigmentation, and scarring,” she said.

Studies have shown the treatment can improve the skin from 25 to 80 percent.

How does the procedure work?

Blood is drawn and spun in a centrifuge at high speeds to separate the red blood cells from the plasma, Malinowski said. The plasma, rich in platelets, is then injected into the client’s face and neck. The platelets release growth factors which stimulate fibroblasts, responsible for the formation of collagen Types I and III, and other skin repairs.

Duration of the improvements will vary based on genetics, general health and nutrition, care of skin, sun exposure, etc., Malinowski said.

“Most experts believe the effects last at least as long as the effects of fillers (i.e. one to two years), and considerably longer in many patients,” she said.

Because Platelet Rich Plasma therapy capitalizes on the healing powers within one's own body, clients never have to worry about product recalls or late onset side effects from introducing a foreign substance into the body.

The most commonly reported side effects include mild bruising from the injection or pain at the site of injection. Because the skin of the face and neck is sensitive, most practitioners will apply a topical anesthetic cream or gel before performing Platelet Rich Plasma injections.

Composed of 40 percent mandelic acid, part of the alpha hydroxy acid group, it is a more tolerable peel, said Kathe Malinowski, lead esthetician and marketing manager for Eterna MedSpa & Laser Vein Center.

“The peel can be used during a sunny season or on sensitive and hyperactive skin because it is a very light, non-invasive peel,” she said.

With the peel, dead skin is sloughed off revealing and promoting new skin cell growth.

Mandelic acid peels can lighten the dark spots left behind after an acne breakout and ease the inflammation brought on by rosacea.

But take note--the treatment takes time.

For best results, Malinowski recommends the peel be done in a series of four.

“They can be done 10 days apart,” she said.

Clients can do up to three Mandelic acid peel series in a year, depending on how the skin responds to the treatments.

Because the peels are so mild, there is little downtime. Clients may notice some redness.

“But generally just right after,” Malinowski said.

And not everyone will experience that side effect.

Microlaser peels can brighten skin, smooth fine lines

 

A skin rejuvenating procedure is giving the boot to fine lines and wrinkles.

With microlaser peels, the outermost layer of the skin is removed. During the healing process, new skin cells form, improving the skin’s overall appearance.

“The peels lighten up hyperpigmentation,” she said.

The state-of-the-art cosmetic laser skin procedure is ideal for the face, neck and chest.

Depending on skin type, condition and improvement goals, the depth of the peel can vary, from four microns to 50 microns.

“Generally four to six microns is a very light peel that will cause minimal peeling,” Malinowski said.

“You get a good peel with a 10 micron,” Malinowski said.

Those who have severe sun damage and a thick stratum corneum, or outermost layer of the skin, would be better suited for a depth of 15 to 20 microns.

“With all the peels you will be red the first day and then tight the next day,” Malinowski said.

She said a depth of four to six microns can take about a day to heal while a depth of 10 microns might take two days. A deeper peel can take a little longer.

For best results, the peels should be done in a series of three, with about six weeks in between each procedure.
